Question:  How do I schedule an evaluation/assessment?

• You would need to come into our office during regular business hours and fill out paperwork to make an appointment. At that time, you will be given the written part of the assessment to take with you to fill out. On the day of the appointment, you would return with the paperwork completely filled out and then meet with the assigned counselor.
