Question:  What do I need to bring to my assessment appointment?

• You would bring in the written part of the assessment that you were given on the day that you made the appointment. You would also bring any documentation of household income to see if you qualify for financial assistance. (Preferred is last 2 months of pay stubs or most recent income tax return.)
